MEDICATION

- ALL STUDENTS who attend Astrocamp, need to have this

"Student Health Form" filled out by their Parents, whether they take medicine or not.

- Astrocamp is able to provide some Over the Counter medicine, such as:

** Cough Drops ** Benadryl ** Pepto Bismol ** Ibuprofen, etc.,

to any student but only if it is selected on their "Student Health Form"

- Parents only need a medical professional to fill out this "Medication Authorization Form" if their child has any daily medication that is normally taken at home and Coolidge does not already have this form on file for things such as:

* Over the Counter Medicine

* Vitamins

* Prescription Medicine

- Students cannot carry these on their own; all dosages need to be accounted for by an authorized personnel.

If students need health attention, minor things will be handled onsite by the program staff.

If any major issues arise, they will call to notify parents/guardians.
